Add "Server-Side" migration documentation covering: - Tool metadata mapping (`_meta["openai/..."]` → `_meta.ui.*`) - Resource metadata (CSP, permissions, domain, border preferences) - Resource MIME type (`text/html+skybridge` → `text/html;profile=mcp-app`) - Before/after code examples using `registerAppTool()` and `registerAppResource()` - Key differences summary including not-yet-implemented features Reorganize existing client-side content under updated heading structure. Should we say that these double-tag the tool and double-serve the resources (for both skybridge and mcp apps styles?)

Hmm, in the context of a "Migrate OAI App to MCP App" guide, I'd lean toward leaving that out, but I don't feel strongly about it.
