Update Plant Mortality tech note

Description of changes

• Highlight the major change - uniform 2% mortality rate has been changed to PFT-dependent variable; add a table showing current r_mort parameter values.
• Restructure the text to make conceptual representation and implementation more explicit.
• Generalize the equations for mortality fluxes leaving vegetation pools, so there are fewer equations now.
• Other minor fixes to improve the readability.
